It is desirable not to miss school; however, we understand that situations may arise where missing school cannot be avoided. Below is a form letter that can be supplied to teachers.
These templates are designed for Avery 2"x"4" forms 5163/8163
USA Hockey provides awards to players for the following-
Zero Award- for a goal keeper playing a complete game without allowing a goal.
Hat Trick Award- for a player scoring 3 goals in a game.
Playmaker Award- for a player registering 3 assists in a game.
Email your D4 Registrar with the following information-
Name of the player.
A copy of the score sheet.
A limit of 1 award per player, per season.
One Goal, Bring a Friend to Hockey, Play Hockey for a Day, and Play Hockey for Free, etc. events require a Special Event Sanction. The steps to process and guidelines for consideration are:
>The host program must apply to the Michigan Registrar for a USA Hockey Special Event Sanction by submitting a brief description of the event: date, time, agenda/schedule/description of the event 14 days prior to the event. This may be done by email to the Michigan Registrar. michregistrar@gmail.com
>After review and approval, the event will be USA Hockey sanctioned.
>All Sanction fees are waived for Grow the Game Events.
>Each participant is required to wear gloves (any kind) and a helmet.
>A completed IMR/Waiver is required for each participant.
>The Host should record all information from the completed IMR/Waiver and keep on file for their use. The completed IMR/Waiver forms should then be forwarded to the USA Hockey Membership Development.
>Properly registered coaches are required to be on the ice.
There are no hard and fast rules as to the length or structure of your event. This provides flexibility to the programs to develop events that fit their needs.
The event should be just that . . . an event for players to try hockey 1, 2 or 3 times – the program could be a one-time event or it could be held (for example) three Saturdays in a row. There might be a program that goes for more than 3 sessions, but each individual participant would be limited to 3 sessions. (Season long sanctions can be issued, but participants are limited to 3 sessions.)
Learn to skate and other programs that run over 3 sessions must register all participants with USA Hockey and their District Associate Registrar.
